Description

Authorize to amend an agreement with Families in Transition Inc., Manchester, NH (originally approved by G&C on 12-3-14, item #15), to provide affordable permanent housing and supportive services to chronically homeless individuals with disabilities, by increasing the price by $2,063 from $156,421 in an amount not to exceed $158,484. Effective upon G&C approval through the original end date of December 31, 2015. 100% Federal Funds.
