Authorize to enter into a retroactive sole source agreement with Southwestern Community Services Inc., Keene, NH, to provide Permanent Housing program services to the chronically homeless, which will facilitate the movement of homeless individuals and families with disabilities to permanent housing and maximum self-sufficiency through the Federal Continuum of Care program in an amount not to exceed $106,687. Effective retroactive to November 1, 2014 through October 31, 2015. 100% Federal Funds.
