Authorize to enter into an agreement with the Community Health Access Network Inc., Newmarket, NH, to coordinate and implement Asthma, Diabetes and Cardiovascular, and Tobacco prevention health care interventions to improve prevention and management of chronic diseases, in an amount not to exceed $628,843. Effective upon G&C approval through June 30, 2016. 100% Federal Funds.
