Description

Authorize to enter into an agreement with Families in Transition Inc., Manchester, NH, to provide affordable permanent housing and supportive services to chronically homeless individuals with disabilities in an amount not to exceed $156,421. Effective January 1, 2015, or upon G&C approval, whichever is later, through December 31, 2015. 100% Federal Funds.
