Authorize to enter into a retroactive sole source amendment to an agreement with the University of Massachusetts Medical School, Worcester, MA, (originally approved by G&C on 6-20-12, item #77), for the provision of disability determination services, by increasing the price by $100,000 from $926,750 to $1,026,750, and by extending the completion date from September 30, 2014 to October 31, 2014. 75% Federal, 25% General Funds.
