Authorize to exercise renewal options with vendors as detailed in letter dated April 28, 2014, (originally approved by G&C on 8-8-12, items #49,50,51 and 52), to provide statewide home visiting services to families in accordance with the Health Families America model, by increasing the price by $1,483,782 from $1,217,491 to $2,701,273, and by extending the completion date from June 30, 2014 to June 30, 2016. 100% Federal Funds.
