Authorize to amend one agreement (originally approved by G&C on 8-14-13, item #41 and 9-4-13, item #59), and enter into two new agreements with multiple vendors as Medical Services Consultants to complete medical eligibility assessments for Medicaid applicants seeking Long Term Care services, by increasing the price in the aggregate by $84,820 from $501,802 to $586,622, and by extending the completion date to June 30, 2015 from June 30, 2014. 75% Federal, 25% General Funds.
