Authorize to amend a contract with Anthem Life Insurance Company, Worthington, OH, (originally approved by G&C on 12-8-10, item #27), to cover recent changes to certain collective bargaining agreements that increases the employer-paid life insurance benefit for employees from $20,000 to $25,000; it is predicated on a range of between 3,000 to 10,000 lives for the employer-paid life insurance provided to full-time employees; by increasing the amount by $60,000 from $643,500 to $703,500. Effective January 1, 2014 through December 31, 2015. 34% General, 21% Federal, 45% Other Funds.
