Description

Authorize to enter into a sole source amendment to an existing contract with The Granite Young Men’s Christian Association, Manchester, NH (originally approved by G&C on 6/24/20, Item #39), to increase the number of healthcare providers referring individuals with or at risk for chronic conditions to health intervention programs, by increasing the price limitation by $760,000 from $1,374,000 to $2,134,000 and by extending the completion date from June 30, 2025 to June 28, 2028. Effective upon G&C approval. 100% Federal Funds Glencliff Home
