Authorize to amend a sole source contract with Cognosante LLC, Scottsdale, AZ (originally approved by G&C on 12-1-04, item #E), for continuation of quality assurance services and expansion of the scope of services in compliance with the Centers for Medicare and Medicaid Services, by increasing the price limitation by $986,400 from $12,270,475 to a new amount not to exceed $13,256,875, and by extending the completion date from December 31, 2014 to June 30, 2015. 90% Federal, 10% General Funds.
