Authorize, for the benefit of the Department of Health and Human Services, to amend a sole source contract with Interactive Voice Applications Inc., Dallas, TX (originally approved by G&C on 6-13-07, item #20), for software enhancements, and to extend the ongoing support, maintenance, and hosting services provided for the email-based Random Moment Sample service, by increasing the amount of $40,000 from $115,000 to $155,000, and by extending the end date from July 1, 2014 to June 30, 2016. 100% Other (Class 027) funds: The Agency Class 27 funds used by the Department of Health and Human Services to reimburse DoIT for this work is 58% General, 42% Federal Funds.
