Description
Authorize the Special Medical Services Section to enter into an agreement with Concord Regional Visiting Nurse Association, Concord, NH, for a Registered Nurse to provide community-based care coordination services to children with special health care needs and their families residing in Merrimack County, in an amount not to exceed $123,612.04. Effective July 1, 2013 or upon G&C approval, whichever is later, through June 30, 2015. 30% Federal, 70% General Funds.
