Authorize the Special Medical Services Section to enter into an agreement with the White Mountain Community Health Center, Conway, NH, to provide family support services for children and adolescents with chronic health conditions and their families, in an amount not to exceed $69,936. Effective July 1, 2013 or upon G&C approval, whichever is later, through June 30, 2015. 100% Federal Funds.
