Description
Authorize to amend an existing agreement with Area Agency of Greater Nashua Inc. d/b/a Gateways Community Services, Nashua, NH, (originally approved by G&C on 12-8-10, item #140), for Financial Management Services support and respite services for caregivers, by decreasing the funding in SFY 2012 by $28,315 and increasing funding in SFY 2013 by $28,315, with the authority to adjust amounts if needed between State Fiscal Years and within price limitation, and amend the related terms of the contract without further approval from G&C. Effective upon G&C approval through June 30, 2013. 100% Federal Funds.
