Authorize to enter into a sole source amendment to an existing contract, which was originally competitively bid, with Westport Healthcare Management, Inc. d/b/a Pacific Health Policy Group, Lake Bluff, IL (originally approved by G&C on 5/19/21, Item #8), to add scope and funding to support CMS-required activities for the newly approved comprehensive Substance Use Disorder Serious Mental Illness and Serious Emotional Disturbance Treatment and Recovery Access Section 1115(a) Research and Demonstration Waiver for community reentry, by increasing the price limitation by $43,616 from $2,065,700 to $2,109,316 with no change to the contract completion date of December 30, 2027. Effective upon G&C approval. 50% Federal Funds, 34% General Funds, 16% Other Funds (New Hampshire Granite Advantage Health Care Trust Fund)
