Description
Authorize the Disability Determination Unit to amend an agreement with Herbert W. Crosby, Ph.D., (originally approved by G&C on 5-9-12, item #40), to provide services as a Medical Disability Determination Reviewer, by increasing the price limitation by an amount not to exceed $9,000 from $20,000 to $29,000. Effective upon G&C approval through June 30, 2013. 75% Federal, 25% General Funds.
