Authorize the Bureau of Population Health and Community Services, Chronic Disease Prevention and Screening Section, Oral Health Program to enter into a sole source agreement with Bi-State Primary Care Association Inc., Bow, NH, to provide access to oral health care for children and adults living in federally designated Dental Health Professional Shortage Areas in rural NH, in an amount not to exceed $939,017. Effective upon G&C approval through August 31, 2015. 100% Federal Funds.
