Authorize to amend a sole source contract with Cognosante LLC Scottsdale, AZ (originally approved by G&C on 12-1-04, late item E), for continuation of quality assurance services and expansion of the scope of services in compliance with the Centers for Medicare and Medicaid Services regulations related to the State’s Medicaid Management Information System project, by increasing the price by $2,663,798 from $9,606,677 to an amount not to exceed $12,270,475, and by extending the completion date from December 31, 2013 to December 31, 2014. Effective upon G&C approval. 90% Federal, 10% General Funds.
