Authorize to hold a series of seven organized meetings, between February 16, 2012 and June 21, 2012, to foster increased employment opportunities for people with mental illness, for a total cost not to exceed $94,361. (2)Further authorize to enter into a sole source agreement with David Lynde, MSW, Bow, NH, to provide training and facilitation services at the meetings above, in an amount not to exceed $15,300. (3)Further authorize payment of $1,225 to the Holiday Inn, Concord, NH, for facility room rental; authorize stipend payments totaling $1,400 to consumers and family members for participation; and authorize payments totaling $76,436 to reimburse Community Mental Health Center for loss of billable staff time and travel. Effective upon G&C approval through June 30, 2012. 100% Federal Funds.
