Authorize the Bureau of Population Health and Community Services, Healthy Eating and Physical Activity Section to amend an agreement with Southwestern Community Services Inc., Keene, NH, (originally approved by G&C on 5-25-11, item #79), to provide Women, Infants and Children clinic improvements, updated health assessment equipment, and training costs, by increasing the price by $15,770 from $694,832 to $710,602. Effective February 1, 2012 or upon G&C approval, whichever is later. 100% Federal Funds.
