Authorize the Bureau of Population Health and Community Services, Healthy Eating and Physical Activity Section to amend an agreement with Community Action Program Belknap-Merrimack Counties inc., Concord, NH, (originally approved by G&C on 6-8-11, item #91), to provide Women, Infants and Children clinic improvements, updated health assessment equipment, and training costs, by increasing the price by $24,332 from $2,085,644 to $2,109,976. Effective February 1, 2012 or upon G&C approval, whichever is later. 100% Federal Funds. 4 Bureau of Homeless and Housing Services
