Description
Authorize to amend an existing contract with Greater Seacoast Community Health, Somersworth, NH, (originally approved by G&C on 1/22/21, item #23), for Family Support Coordinator service statewide, by exercising a contract renewal option by increasing the price limitation by $75,000 from $150,000 to $225,000 and extending the completion date from June 30, 2022 to June 30, 2023. Effective upon G&C approval or July 1, 2022 whichever is later. 100% Other Funds (Governor’s Commission).
