Authorize to amend an existing agreement with JSI Research & Training Institute, Inc., d/b/a Community Health Institute, Bow, NH, (originally approved by G&C on 3/13/19, item #12) to continue providing professional support services to the Department’s Oral Health Program in order to implement activities funded through federal grants, including enhanced access to preventive and reparative dental treatment for individuals in statewide community-based and school-based oral health programs, by increasing the price limitation by $375,000 from $4,695,833 to $5,070,833 with no changes to the contract completion date of August 31, 2023. Effective upon G&C approval. 100% Federal Funds.
