Description
Authorize to amend an existing contract with County of Cheshire (originally approved by G&C on 9/23/20 item #14), for the provision of Care Management Entity services and enhanced Care Coordination through the Families and Systems Together Forward program, by increasing the total price limitation by $775,000 from $3,500,000 to $4,275,000 with no change to the contract completion date of June 30, 2023. Effective upon G&C approval. 17% Federal, 83% General Funds.
