Authorize to enter into a retroactive sole source amendment to an existing contract with Accuity Asset Verification Services, Evanston, IL, (originally approved by G&C on 3/22/17, item #16) to continue electronic asset verification services for applicants and Medicaid recipients, by increasing the price limitation by $139,854 from $1,689,153 to $1,829,007 and by extending the completion date from February 28, 2022 to December 31, 2022. 50% Federal, 50% General Funds.
