Authorize to enter into a sole source retroactive amendment to an existing contract with On-Site medical Services, LLC, Charlestown, NH, (informational item #J, G&C 4/21/21) for the provision of COVID-19 vaccinations and boosters to individuals across the state, as requested by the Department, by exercising a contract renewal option by increasing the total price limitation by $3,400,000 from $31,882,944 to $35,282,944 and extending the completion dates from March 31, 2022 to June 30, 2022. 100% Federal Funds.
