Description

Authorize to enter into a sole source contract with Evident Change, Madison, WI, in the amount of $269,300 to review and revise the Structured Decision Making assessment tools used by the Bureau of Elderly Services serving individuals in the Adult Protective Services Program. Effective upon G&C approval through June 30, 2023, with the option to renew for up to two additional years. 100% Federal Funds.
