Description
Authorize to enter into an amendment to an existing contract with Fivebase, LLC, Hampton, NJ, (originally approved by G&C on 8/18/21, item #36), for continuous support for the NH Hospital electronic health record system, by exercising a contract renewal option by increasing the price limitation by $470,000 from $162,500 to $632,500 and extending the completion date from August 31, 2022 to August 31, 2023. Effective retroactive to April 1, 2022. 100% Other Funds (Inter-Agency).
