Description
Authorize to enter into a contract with Amoskeag Health, Manchester, NH, in the amount of $280,000 for care coordination for children with medical complexity and their families in Hillsborough, Strafford, and Rockingham Counties. Effective upon G&C approval or July 1, 2022 whichever is later, through June 30, 2023, with the option to renew for up to 2 additional years. 25% Federal, 75% General Funds.
