Description
Authorize to amend an existing contract with NFI North, Inc., (originally approved by G&C on 6/21/17, item #39B), for the provision of Care Management Entity services and Enhanced Care Coordination through the Families and Systems Together Forward program, by increasing the total price limitation by $900,000 from $4,752,160 to $5,652,160 with no change to the contract completion dates of June 30, 2023. Effective upon G&C approval. 16% Federal, 84% General Funds.
