Authorize to amend an existing cooperative agreement with University of new Hampshire/Institute on Disability, Durham, NH, (originally approved by G&C on 6/20/18, item #22A), to provide support, technical assistance, education and training that promote best practice in services provided to individuals with developmental disabilities, acquired brain disorders and individuals served by community based developmental services system, through community services and the Department, by exercising a renewal option by increasing the price limitation by $431,896 from $1,006,041 to $1,437,937 and by extending the completion date from June 30, 2022 to June 30, 2024. Effective upon G&C approval. 21% Federal, 79% General Funds.
