Authorize to enter into a contract with Lamprey Health Care, Inc., Newmarket, NH, in the amount of $200,922 for chronic disease health promotion programs for individuals with a chronic condition or family caregivers, statewide. Effective October 1, 2022 through September 30, 2025, upon G&C approval, with the option to renew for up to two (2) additional years. 100% Federal Funds.
