Description
Authorize to enter into a sole source amendment to an existing contract with Granite VNA, Inc., Concord, NH (originally approved by G&C on 2/8/23, Item #28), for foot care services to patients at New Hampshire Hospital, by exercising a contract renewal option by increasing the price limitation by $19,800 from $16,000 to $35,800 and extending the completion date from June 30, 2025 to June 30, 2027. Effective July 1, 2025, upon G&C approval. 30% General Funds, 70% Other Funds (Provider Fees and Intra-agency Transfers)
