Authorize to enter into a sole source amendment to an existing grant agreement with A.W. Holdings, Inc., Fort Wayne, IN (originally approved by G&C on 10/18/23, Item #16), for additional funding to support housing for individuals with a Developmental Disability or Acquired Brain Disorder who are receiving services under a Home and Community Based 1915c Waiver, by increasing the price limitation by $1,000,000 from $4,000,000 to $5,000,000 with no change to the completion date as detailed in the letter dated December 3, 2024. Effective upon G&C approval. 100% Federal Funds
