Description

Authorize to enter into a retroactive sole source contract with the City of Nashua, NH, in the amount of $381,000, to fund Community Health Workers to connect individuals to health and social services resources in the Nashua area. Effective retroactive to July 1, 2024, upon G&C approval through June 30, 2026, with the option to renew for up to two additional years. 100% Federal Funds
