Authorize to enter into a sole source amendment to an existing contract with Thrive Survivor Support Center, Concord, NH (originally approved by G&C on 9/25/24, Item #27, archived document ), to add funding and modify scope for the continued provision of Continuum of Care housing services, by increasing the price limitation by $14,625 from $712,830 to $727,455, and extending the completion date from September 30, 2029 to November 30, 2029. Effective upon G&C approval. 100% Federal Funds
