Authorize to enter into a sole source contract with Easter Seals New Hampshire, Inc., Manchester, NH, in the amount of $441,041, inclusive of a shared price limitation of $160,000, for a health care coordination program to enhance the system of care with community-based, family-centered care coordination for children with special health care needs. Effective upon G&C approval through June 30, 2026, with the option to renew for up to two additional years. 52.82% Federal Funds, 47.18% General Funds
