Description
Authorize to amend an existing contract with Foundation for Healthy Communities, Concord, NH (originally approved by G&C on 6/29/22, Item #32), to continue to serve as the Administrative Lead Organization for the statewide Health Care Coalition for emergency preparedness, response, and recovery, by exercising a contract renewal option by increasing the price limitation by $1,307,035 from $5,423,436 to $6,730,471 and by extending the completion date from June 30, 2024 to June 30, 2026. Effective July 1, 2024, upon G&C approval. 100% Federal Funds
