Authorize to enter into a sole source contract amendment with Windsor Solutions, Inc., Portland, OR (originally approved by G&C on 6/14/14, Item #15), by increasing the price limitation by $1,595,472.33 from $2,893,932 to $4,489,404.33 and by extending the completion date from June 30, 2024 to June 30, 2029, for the purpose of ongoing maintenance and support of the State’s primary electronic forms software platform. Effective upon G&C approval through June 30, 2029. 100% Other (Agency Class 27) The Agencies Class 027 used to reimburse DoIT is 22% Federal Funds, 37% General Funds, 41% Other Funds
