Description

Authorize to enter into a sole source amendment to an existing contract with Bi-State Primary Care Association, Inc., Bow, NH (originally approved by G&C on 11/10/21, Item #16B), to continue immunization-related activities and sustain the workforce of Community Health Workers in Community Health Centers, by exercising a contract renewal by increasing the price limitation by $400,000 from $6,553,032 to $6,953,032 and extending the completion date from June 30, 2024 to June 30, 2025. Effective July 1, 2024, upon G&C approval. 100% Federal Funds
