Authorize to enter into a sole source amendment to an existing contract with North Country Health Consortium, Littleton, NH (originally approved by G&C on 6/1/22, Item #24), to expand Community Health Worker training and Coalition support services by exercising a contract renewal by increasing the price limitation by $350,000 from $1,500,000 to $1,850,000 and by extending the completion date from June 30, 2024 to June 30, 2025. Effective upon G&C approval. 86% Federal Funds, 14% General Funds
