Description

Authorize to enter into a sole source amendment to an existing agreement with Suzanne Appleton, Hebron, NH (originally approved by G&C on 6/1/22, Item #16), for the continued provision of massage therapy services to residents of Glencliff Home, by exercising a contract renewal option by increasing the price limitation by $15,900 from $15,000 to $30,900 and extending the completion date from June 30, 2024 to June 30, 2026. Effective July 1, 2024, upon G&C approval. 33% General Funds, 67% Other Funds (Agency Income)
