Authorize to enter into a sole source amendment to an existing contract with Fivebase, LLC, Hampton, NJ (originally approved by G&C on 8/18/21, Item #36), to continue providing information technology support for New Hampshire Hospital’s electronic health record system, to provide direct support and resources in support of the hospital’s migration to the next generation electronic health record system, AvatarNX, and to integrate AvatarNX data with the Department’s Electronic Data Interchange system, by increasing the price limitation by $2,864,756 from $932,500 to $3,797,256 and extending the completion date from August 31, 2024 to June 30, 2028.
