Description

Authorize to amend an existing contract with Stephen Francis Caldwell, DDS, MS, Derry, NH (originally approved by G&C on 4/20/22, Item #14), for continuation of orthodontic dental consulting services, by extending the completion date from June 30, 2024 to June 30, 2026 and increasing the price limitation by $191,360 from $191,360 to $382,720. Effective July 1, 2024, upon G&C approval. 50% Federal Funds, 50% General Funds
