Description

Authorize to enter into a grant agreement with A.W. Holdings, LLC, Fort Wayne, IN, in the amount of $4,000,000 to increase housing capacity by forty beds for individuals who are eligible for one of NH’s Home and Community Based 1915c Waivers. Effective upon G&C approval, to remain in effect for no less than eight years from the date the last home is placed in service. 100% Federal Funds
