Authorize to enter into a grant agreement with Farmsteads of New England, Inc., Hillsborough, NH in the amount of $1,300,000, to increase housing capacity by nine beds for three individuals who are eligible for one of NH’s Home and Community Based 1915c Waivers and six individuals who are disabled and are experiencing homelessness and eligible to receive services through the Home Community Based Services State Plan 1915(i). Effective upon G&C approval, to remain in effect for no less than eight years from the date the housing unit is placed in service. 100% Federal Funds
