Authorize to enter into a grant agreement with Next Steps Community Services, LLC, Madison, NH, in the amount of $1,600,000 to increase housing capacity by sixteen beds for individuals who are eligible for one of NH’s Home and Community Based 1915c Waivers. Effective upon G&C approval, to remain in effect for no less than eight years from the date the housing unit is placed in service. 100% Federal Funds
