Authorize to enter into a contract with Public Consulting Group LLC, Boston, MA, in the amount of $1,637,117 for facilitation, scheduling and completion of the Supports Intensity Scale-Adult® assessment for all individuals who are receiving, and who plan to receive services through the NH Development Disability 1915(c) waiver in accordance with He-M 500, part 503 and part 522. Effective upon G&C approval through June 30, 2025, with the option to renew for up to four additional years. 100% General Funds
