Description
Authorize to enter into a sole source amendment to an existing agreement with Denise Stuart, Plymouth, NH (originally approved by G&C on 10/19/22, Item #19), for dental hygienist services to residents of Glencliff Home, by exercising a contract renewal option by increasing the price limitation by $7,500 from $7,500 to $15,000 and extending the completion date from September 30, 2023 to September 30, 2024. Effective October 1, 2023 or upon G&C approval, whichever is later. 21.58% General Funds, 78.42% Other Funds (Agency Income)
