Authorize to enter into a retroactive sole source amendment to an existing contract with the Disability Rights Center, Concord, NH (originally approved by G&C on 3/9/22, Item #41), for the purpose of building and maintaining a coalition of people with disabilities, family members, advocates, service providers and others to identify barriers and opportunities to expand the use of supported decision making in NH by increasing the price limitation by $12,500 from $75,000 to $87,500 and by extending the completion date from June 30, 2023 to June 30, 2024. Effective upon G&C approval. 100% Federal Funds
